Health checkups and certificate issuance
Student ID and Health Checkup Procedures

Please submit a copy of your workplace health checkup results to the Nurse's Office of Safety and Health Organization by the end of January of the relevant academic year. CU health checkupy is not required.

If your student ID number changes, you will not be able to make reservations, answer the medical questionnaire, or receive the checkup results. Please contact the Nurse's Office of SHO as soon as possible. Email: info-hsc@office.chiba-u.jp

If your student ID number remains the same, you can use your current student ID to take the health checkup.

Yes, you can. However, complete the medical questionnaire and make a reservation in advance are needed. Please use a device connected to the campus LAN to complete these steps beforehand.

Please take the general periodic health checkup first. You will be informed of another date for blood testing. Reservations for blood testing are not available, so you may need to wait for cancellations. If you wish to make a reservation, please contact the Nurse's Office .
If you are a university employee working 20 hours or more per week or employed at the affiliated hospital, you are required to take the staff health checkup, which takes priority. Reservations for the staff health checkup will begin in mid-May. If you wish to take the student health checkup due to scheduling reasons, please consult with the Nurse's Office.
If you will return to Japan by mid-June, please contact the Nurse's Office by the end of April. If he/she returns after that, arrangements will be made for you to take the health checkup during the fall session. Details will be provided by the Academic Affairs Office around October, so please wait for further instructions.
Regarding Medical Questionnaire Input and appointment scheduling

If you are employed as a staff member of this university, a health checkup plan may be created around mid-May. For more details, please refer to “Q7” and “A7.” If you do not have a status of staff, please contact the Nurse’s Office.
You can revice your answers to the medical questionnaire as needed, up until right before your health checkup.
If you could not attend the health checkup, the appointment will be canceled after the checkup period ends (usually around 17:00). Please reschedule afterward. You can revise your reservation as many times as needed, up until two days before your scheduled appointment.
For example, your appointment is on May 5, you can make changes online until 23:59 on May 3.Devices connected to the CU network are available at CUDTEC, library and Nurse's office. Wi-Fi is also available at those places. For more details, please refer to the CUDTEC website (accessible only within the campus network).
Wireless LAN ServiceAppointments are scheduled in 15-minute intervals. If your reservation is at 10:00, please arrive at the venue by 10:15
Regarding Health Checkup Results and Certificate Issuance
The mental health questionnaire (psychiatric evaluation) may result in a classification of “C1: Follow-up observation” or “D2: Re-examination required.” If the result is “D2: Re-examination required,” the advisory message will state: “Based on the results of the mental health questionnaire, a more detailed assessment is necessary. You will receive guidance by email in a few months, so please make sure to respond.” Alternatively, you can check your overall assessment not through “Download Report,” but by the “chronologically” feature.
The result of the mental health questionnaire is not included in the official health examination certificate
Please submit the certificate issued from the automated certificate issuance machine. If a specific format is required, please apply at the office on the first floor of SHO or Nurse's office.
The results for non-regular students will be available on the web about two weeks after the examination. Results for special health check-up and for examinations held at the Matsudo or Inohana campuses will be available in about four weeks.
If it is unable to view the general health check-up results on the web, you can apply for a health certificate at the General Administration Office on the first floor of SHO. It will be issued about one week after the application.Applications for issuing certificates using designated forms will be accepted starting from the day after the last day of the Nishi-Chiba campus student health checkup.
“General Periodic Health Checkup Certificate” and the “Special Health Checkup Certificate” can be issued. In addition, the results of health examinations conducted at this university can be transcribed onto designated forms provided by employers or institutions for study abroad, in either Japanese or English. There is no fee for this service. Please note that we cannot certify examinations that were not conducted at Chiba University.
As a general rule, it cannot be issued.
Regarding Secondary (Follow-up) Examinations
Please consult the Nurse's office, SHO as soon as possible after the health checkup, preferably within one week.
You may visit an external medical institution without waiting for the scheduled follow-up examination. In that case, please submit the examination results-preferably in written form- to the Nurse's Office, SHO (or the first-floor office).
