Issuance of Health Checkup Certificates.

Health Checkup Certificates will be issued only to students who have undergone this year's annual health checkup. Students who did not participate will not be eligible to receive a certificate.
If you did not undergo this year's health checkup but require a certificate, please visit a nearby medical institution to receive an examination.

Regarding Certificate Issuance by Automated Certificate Issuance Machine
Locations of the Automated Certificate Issuance Machines:
- Nishi-Chiba Campus: Student Support Division (2 machines)
- Inohana Campus: 1st Floor Hall, Research Bldg of Medical Sciences
- Matsudo Campus: In front of the Office of the Faculty of Horticulture

| Purpose | Primarily for: (1) Employment (2) Further education (3) Scholarship applications |
| Issuance starts | Certificates are scheduled to be issued from mid-May.。 Note: Research students cannot obtain certificates from the issuing machines. Notification will be sent to the individual by their faculty, approximately one month after the health checkup. |

If any trouble with the certificate issuing machine, recovery may take some time.
We recommend using the machine well in advance 5 to 7 days. - Request
- Due to expected congestion, please issue only the number of copies you need.
Regarding Certificate Issuance by SHO (From Late May every academic year)
Certificates will be issued by SHO only in the following cases.
For all other cases, please use the issuing machine.
- Designated form is required.
- Research students (approximately one month after the medical examination)
Certificates will be issued upon submission of the application form.
The issuance may take several days, so please apply at least one week before the submission deadline.
If new tests such as blood work, are required and the SHO determines that issuance is not possible, your application may be returned.

| About the Application Form | Application forms are available in front of the 1st floor of health center, SHO. (For students in the Matsudo and Inohana campuses, please contact the Student Affairs or Academic Affairs Office of your respective faculty.) Please ensure that all required fields are completed before submission, especially your contact information. |

Please understand that during the annual health checkup period (through the end of June), certificate issuance may take longer than usual.
