Issuance of Health Checkup Certificates for Overseas Study or Training
※Chiba University (CU) certificate issuance services are available only to individuals who have undergone the general annual health checkup for the relevant academic year.
For overseas study programs or international training, host institutions may require the submission of English certificates such as 'vaccination records', 'serological antibody test results', and 'general health checkup certificates'. To support this, Safety and Health Organization provides English certificates free of charge upon request from students and staff.
The required certificate contents may vary depending on your destination (country or region), host university, and duration of stay.
Please consult SHO as early as possible. Please be sure to bring any relevant documentation, such as:
- the required certificate format or content guidelines
- your Maternal and Child Health Handbook
- the university-issued vaccination record booklet
- any other immunization records you may have.
After consultation, and if you need to receive the required vaccinations and undergo antibody testing at an external medical institution.
Please note that this process may take at least 2 to 3 months in many cases.
Once these steps are completed, the English health certificate will be issued by the medical institution you went.
Vaccination and testing fees must be paid at your own expense.

- If you plan to study abroad within one month of your consultation, please be aware that due to vaccination scheduling,
it may not be possible to issue the required certificates in time.
To ensure timely submission, please begin preparations at least two months in advance.
