【Contents of Health Checkup】Before taking health checkup ・You will receive an email (@chiba-u.jp) to start your medical checkup appointment.
・【Hm-neo】Health Support System Please complete the questionnaire and make a reservation.On the health checkup day. ■Preparation
・Student/Staff ID card (If your card have magnetic problem, please go to the Student Services Division or your department's office in advance to have it replaced.)
・Plain T-shirt.No loaner shirts available.
・Glasses or contact lenses if needed■Notes
Please refrain from:
・Using the rest room immediately before the urine test.
・Excessive eating and drinking the night before and on the day of the checkup.
・Doing rigorous exercise right before the examination.
・Taking vitamins supplements from the day before the checkup as they may
affect the results.
Do not attend if you feel unwell.
Afterwards ・You will receive an email confirming the result. (@chiba-u.jp)
・Please check the result from Health Support System ”HM-neo”.Chest X-ray Please check in at the reception, then enter the room for your X-ray scan.
Changing area will be prepared. *Tie up your hair to ensure it is clear of your shoulders.
*Wear plain T-shirt. Please remove underwear including bra top and necklaces.Consultation A medical consultation will be provided at the infirmary.

*If your hair is tied up, please untie it before the measurement.*Those who require a secondary health examination will be notified on the same day.
- Person subject to Chest X-ray

- 1st year Student (Undergraduates, Transfer, Research etc.)
- Undergraduate and Graduate School Students of Medicine, Pharmacy, Nursing, Education, Medicine and Pharmaceutical Sciences, who are planning to go for practical training during the year.
- Students who are planning to go on a training course within the current academic year in faculties and graduate schools other than those listed above.
- International student
- Those who had chest X-ray findings in the previous year.
- Those who have respiratory symptoms (cough, phlegm, etc.) or circulatory symptoms (palpitations, chest pain, etc.)
- Those who have been determined to require an examination by a doctor.
- Persons subject to special health examinations (organic solvents, specified chemical substances, persons handling pathogens, persons engaged in genetic modification experiments, etc.)
- Students who have not undergone a health checkup in Chiba University.
- Those with reasons other than that mentioned above.
- For those who are undergoing a health checkup equivalent to a regular health checkup (comprehensive medical checkup, etc.)

If you are planning to undergo a medical examination equivalent to annual health check (such as comprehensive medical check-up), and would like to have the examination replaced by submitting the results report, please take steps as follows:
・Mutual aid association member employees
Regarding the application for comprehensive checkups through the Mutual Aid Society's website and the submission of the results to the employing institution (Chiba-University) by the medical institution outside..

If you have not agreed to the handling of your personal information, ⇒ Required to submit
If you are required to submit the results, please enclose all the copies into an envelope, and write “Results of physical checkup”, “affiliation/faculty”, and “name” clearly on the front side, please write or note “student/staff ID number” inside, and send to Nurse's office, SHO (by the end of January, 2026).。
The copy should include
・Name
・Date of birth
・Date of health examination
・Name of medical institution
・Health Examination Results and Assessment (If possible, please include the evaluation category assigned by medical institution and any guidance or instructions from the doctor.)

・Please print the form, fill in the required information, and submit it to the SHO.
・Send by campus mail or other postal services.
Note: If you have undergone a follow-up chest X-ray examination at the SHO, submission of this form is not required.
